Operative Transhepatic Hepaticostomy



Abstract
Since the first presentation of the hepaticostomy by Otto Goetz in 1951, hepaticostomies have been used restrictedly after surgery of the common bile duct for stent, decompression or palliation from obstructing malignant tumors of the biliary
tree
and
others.
Over a nine year peroid from March 1981 to December 1989, operative transhepatic hepaticostomy has been used in 43 cases of gallstone disease and 7 cases of malignant biliary obstruction at Masan Koryo General Hospital. Overall stone removal rate
for
retained intrahepatic stones has been reached to 79.07% by irrigation using normal saline with hydrogen peroxide through the hepaticostomy tube. The hepaticostomy was attributed to 6% of total complication cases who were applied to this
procedure.
The
surgical hepaticostomy can be used on the biliary surgical cases as an alternative method of biliary drainage to T-tube and for the purpose of irrigation and postoperative management of the biliary tract problems.
